Eligibility for Medical and Dental Insurance
Legal Separation/Divorce: Spouses are ineligible for state-subsidized medical and dental benefits as of the first of the month following the date of the legal separation. Former spouses are ineligible for state subsidized medical and dental benefits as of the first of the month following the date of the divorce. You are required to report the change in marital status with 31 days. Continuation of coverage provisions are described in General Notice No. 2012-3 from the Office of Policy Management and the Office of Labor Relations. You will need a personalized enrollment form to report your change in marital status and indicate whether or not you wish to continue the unsubsidized coverage. You order the personalized form from Benefits: Request for Personalized Benefits Form. You must return the completed form within 31 days following your legal separation or divorce. If you are enrolled and wish to change your beneficiary, you must request a form:

If you wish to change your tax withholding to reflect the change in your marital status and deductions go to Employee Self Service.
If you wish to change the account to which you are directly depositing your pay check go to Employee Self Service.

If the change in your marital status is causing you problems on or off the job, call an Employee Assistance Program (EAP) representative for free confidential counseling and referral services. Information is available at the EAP website.
